Privacy & Your Rights

The provision of quality health care is our principal concern.  Your doctor regards health information as confidential and will only collect this information with patient consent. 

Your medical record is a confidential document.  It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff. 

We abide by the ten National Privacy Principles, available at  https://www.oaic.gov.au

TRANSFER OF MEDICAL RECORDS

​If you would like to access your medical records, have them sent to us from another practice, or have us send them to your new practice, there are several steps that must be taken:

a.   To access your medical records - call and speak with the Practice Manager or book in with your doctor who will facilitate your access. (Please read our policy on accessing your medical records for more information).

​b.   To have your records sent to us - download our Transfer of Medical Records form, fill it in and return it to us. We will then contact your previous clinic to request the transfer. Medical clinics are allowed to charge a reasonable fee for the transfer of medical records so please be aware that your previous clinic might charge you.

​c.   To have your records sent to your new doctor - ask your new clinic for their Transfer of Medical Records form, complete the form and either send it to us or have their receptionist send it on your behalf. In most cases we do not charge for the transfer of medical records from our clinic.
The only time we charge patients is if the records need to be printed as a hard copy or sent on a disk.
